IRS Form W-9: Request for Taxpayer Identification Number and Certification

IRS Form W-9, Request for Taxpayer Identification Number and Certification, is used to certify the taxpayer identification number (TIN) or Federal Employer Identification Number (FEIN), type of taxpayer, and tax status. UCSD needs to be concerned with W-9s for both payers and payees.

W-9s for payers to UC San Diego

Your department might receive a W-9 request from payers so that they can properly report information in connection with miscellaneous payments to the university (for interest, dividends, rents, royalties, etc.). If you receive a request from a payer for a W-9, for most purposes, submit the standard UCSD form W-9, for FEIN 956006144 (PDF). For US income tax purposes payees will be asked to provide their citizenship status, legal name, address and SSN when registering via PaymentWorks.

The signature authority for W-9s have been delegated to departments.  As long as you are a US citizen or US resident alien and a UC San Diego employee, you or your supervisor, may sign and date the W-9.

W-9s for gifts to UC San Diego

Some donors will request a W-9 or vendor setup to set up payment for their gifts. Advancement Services will need to complete the setup and provide the W-9. Submit a Services and Support Ticket to About:  Gift Funds, Related to: Gift Fund Inquiries; More Specifically: Gift Fund Questions. Be sure to include the fund number the gift will go to, along with any related gift agreement, correspondence or other backup related to the gift. Someone from Gift & Foundation Accounting or Gift Services will provide the W-9 or complete the vendor registration.

The following tax-id’s are valid for gifts:

  • Fund starts with A-L: UC San Diego Foundation: 95-2872494
  • Fund starts with M-Z: University of California Regents: 94-3067788
