How to Process a Lost or Damaged Shipment
Contact Logistics immediately through UC San Diego Services & Support
- Logistics Request Form number
- Shipping date
- For damaged shippments Logistics will:
- Call the person who received the item and instruct that person to keep all packaging materials and paperwork.
- Call the carrier, request an inspection of the package, and submit an Intent to File a Claim form.
- After the inspection, file a claim with the carrier and forward the inspection report and a claim report to UCSD Risk Management.
- For lost shippments Logistics will:
- Call the carrier and place a trace on the item.
- File a claim with the carrier if the item can't be found.
- Upon conclusion of the claim, forward a claim report to UCSD Risk Management.
- Risk Management will contact your department and the person who authorized the shipment.
