How to Process a Lost or Damaged Shipment

Contact Logistics immediately through UC San Diego Services & Support

  • Logistics Request Form number
  • Shipping date
  • For damaged shippments Logistics will:
    • Call the person who received the item and instruct that person to keep all packaging materials and paperwork.
    • Call the carrier, request an inspection of the package, and submit an Intent to File a Claim form.
    • After the inspection, file a claim with the carrier and forward the inspection report and a claim report to UCSD Risk Management.
  • For lost shippments Logistics will:
    • Call the carrier and place a trace on the item.
    • File a claim with the carrier if the item can't be found.
    • Upon conclusion of the claim, forward a claim report to UCSD Risk Management.
  • Risk Management will contact your department and the person who authorized the shipment.
