How to Complete a Customer Service Request (CSR) Form

Find out how to complete a Customer Service Request (CSR).

CSRs are used to request telecommunication services for La Jolla on- and off-campus offices only. These include desk phone installations, Ethernet wiring, pager activations, and radio orders.

Access the CSR form

Add contact information

  • Provide the following contact information:
    • Department name
    • Project Task form (PTF) number *
    • Mail code
    • Service contact name, phone number, and e-mail address
    • Fiscal contact name, phone number, and e-mail address

Note: Make sure the PTF number is active and valid. Check with your department's business office if you're unsure.

Add service information.

  • Select a service.
    • Click the drop-down menu and select a service. Note: You can select only 1 service in this field. To request more than 1 service, use the text box to describe all the services you need instead of completing an additional CSR (see step 3d).
  • Select a service analyst (optional).
    • To specify your service analyst, use the drop-down menu to select one.
  • Request a service date (optional).
    • To specify a service date, enter a date at least 1 working day after the date you complete the CSR.
  • Enter a detailed description of the service requested.
    • If your request is urgent, make sure to note it in your description. Any additional requests can be made in this box as well.

Submit the request.

  • Click Submit request and receive an e-mail confirmation. (Note: The fiscal contact will also receive an e-mail confirmation.)
  • A representative from Telecom will contact you within 48 hours to schedule the service.
  • To check the status of a request, go to CSR Status.
