Instructions for candidate

1. Discuss your intention and desire to apply for the Lean Bench Program with your supervisor.
2. Complete the form below to submit your application (a copy of the application will be sent to your supervisor).
3. Request your supervisor to complete and submit the following endorsement as a supplement to your application.

Instructions for supervisor

1. Download the Supervisor Endorsement supplement (Word file).
2. Follow instructions within the document and submit to leanbench@ucsd.edu by the deadline.

Selection process

• The review committee will consider experience level, statement of interest, supervisor support and availability of the applicant.
• Preference will be given to those with Lean Six Sigma Green or Black Belt certification, or equivalent experience.
• The review committee will make recommendations to the Enterprise Information Systems Governance Committee, as required.
