Incident Report for Students & Visitors

Complete this incident report when student/visitor injuries occur and/or student/visitor-owned property is damaged.

• If you want to report safety concerns and near misses only, complete a Safety Concern and Near Miss form instead.
• If you are a UC San Diego employee and want to report a work-related injury, use the Employee Occupational Incident Report.
• If you are a supervisor and want to report an employee's work related injury, use the Supervisor Occupational Incident Report.

Upon submitting an Incident Report for Students and Visitors, you should receive a confirmation email from UC San Diego Risk Management that your incident report has been submitted. Please email Risk Management at EHSRisk@ucsd.edu if you do not receive an email confirmation.
