Disability Counseling and Consulting (NEW PAGE)

Disability Counseling and Consulting (DCC) provides disability management and job accommodation consultation services to UC San Diego faculty and staff.

UC San Diego faculty and staff who have a medical condition (physical or mental) that interferes with their ability to work or return to work can find resources below.

Start Here – Accommodation Services Log

Use the UC San Diego Accommodation Services Log to request a reasonable accommodation or workplace adjustment, or to document one that has been provided. When you open the log, select “Disability Accommodations.” You’ll see the below two options. Once you understand your request type, access the Accomodation Services Log.

Request a reasonable accommodation or workplace adjustment

Choose this option if you need support or changes in your job duties, schedule, equipment, or work environment due to a medical condition.

I want to request a Reasonable Accommodation or Workplace Adjustment

Document an accommodation

Choose this option if you are a supervisor, HR practitioner, or employee who needs to record an accommodation that has already been provided.

Submitting documentation

After completing the Accommodation Services Log, you may need to submit documentation from your licensed treatment provider describing any functional limitations. Your diagnosis is not required.

 

Submit documentation securely to DCC at hrdcc@ucsd.edu or through your assigned Disability Counselor.
