Disability Counseling Support Services
Disability Counseling and Consulting (DCC) is the division of Human Resources responsible for providing disability management and job accommodation consultation services to UC San Diego faculty and staff who have a medical condition (physical or mental) that interferes with their ability to work or return to work.
ADA Coordinator
The UC San Diego ADA Coordinator monitors compliance with the Americans with Disabilities Act to address physical accessibility matters and partners with campus constituents to chair the ADA Title II Committee.

Lincoln Financial Disability Income
Lincoln Financial administers UC San Diego’s claims for Basic Short Term Disability, Voluntary Short Term Disability, and Voluntary Long Term Disability.

Reasonable Accommodations and the Interactive Process
Accommodations Services Log
UC San Diego is required to engage in the Interactive Process and document job accommodations. Please note that completing this form alone should not replace the formal Interactive Process and DCC should be contacted if questions or complex matters arise.
In addition to completing this log, please submit written documentation from your licensed treatment provider that confirms an underlying health condition and identifies any functional limitations as a result of the health condition that render you unable to effectively perform one or more essential functions of your position. Your diagnosis is not being requested.
Documentation should be submitted to hrdcc@ucsd.edu or to your assigned Disability Counselor.
HR Contacts/Supervisors: If you have provided a reasonable accommodation (i.e. ergonomic equipment, modified work schedule, etc.) independent of DCC consultation, it must be registered on the above Accommodations Services Log.
