Forms and templates used to develop recharge rates and requests pertaining to SSEAs.

SSEA Proposal Development

Complete the proposal forms and any other supplemental documents necessary for the approval of the activity.

  1. Recharge Proposal Workbook - Please include all rates in proposal unless BFS-FAO has approved a partial rate proposal. This is revised regularly; please download a new copy from this site for each submission to ensure use of the most recent update.
  2. Proposal Functional Review - To be completed by your VC Office, optional at this time.
  3. UBIT Questionnaire - Only complete for new establishing operations.
