Learn how to donate time to the catastrophic leave program.

Donating to the Catastrophic Leave Donation Program is easy and will make a difference in the life of an eligible colleague.

  1. Review the Catastrophic Leave Donation Program information (PDF) and determine if you are eligible to donate vacation hours.
  2. Complete the Donor Form via DocuSign.
  3. Submit to Department Head or designee for signature and Department HR Contact for completion.
  4. Send the completed form via email to Disability Counseling & Consulting or via hard copy to MC 0944. Be sure to retain a copy for your records.

Leave donations are not tax deductible.
