How to Submit Course Material Adoption Forms

Instructors, follow the guide below to submit course materials adoption forms to the UC San Diego Bookstore.
1. Review adoption request memo and forms and begin determining next-term adoptions.
- The Bookstore's Course Materials department sends the requests for course materials adoptions during Week 2 of the current quarter.
Note: See course materials submission dates for the year.
2. Submit next-term course materials adoptions before the submission request date (typically end of Week 3).
- To submit electronic adoptions or download forms to print, use the Bookstore's course materials adoption form.
- To submit paper course materials adoptions, mail forms (PDF) to the UC San Diego Bookstore Course Materials Department, Mail Code 0008, or fax to (858) 822-0261.
- To submit custom course materials (readers, lab manuals, out-of-print books, syllabi, original works, etc.), contact the Custom Course Materials office, custom@ad.ucsd.edu or (858) 534-7963, or use the links to the online or printed forms.
- To adopt non-printed course materials, such as art supplies or lab notebooks, list the items on the online or printed course materials adoption form and contact the Bookstore's supplies buyer, Steve Kalchik, (858) 534-0124.
Note: The Bookstore accepts adoptions at any time and will make every effort to have all titles available on or before the first day of classes. However, late adoptions are subject to publisher out-of-stock conditions and other factors that may cause delays.
3. Submit requests for instructor's copies (also called desk copies).
- Submit desk-copy requests to publishers during Week 4 of the current quarter for books needed next quarter.
- Find out how to order desk copies at the Desk Copies and Local Publisher Representatives page of the Bookstore website. You can also link to additional publishers and download a request form from this page.
- E-mail, fax, or send requests for desk copies to publishers.
