SSA Proposal Development

Complete the proposal forms and any other supplemental documents necessary for the approval of the activity.

1. Recharge Proposal Workbook - Please include all rates in the proposal unless BFS-FAO has approved a partial rate proposal. This is revised regularly; please download a new copy from this site for each submission to ensure use of the most recent update. Last updated: 09/01/2026
2. Proposal Functional Review - To be completed by your VC Office, optional at this time.
3. UBIT Questionnaire - Only complete for new establishing operations.
