About the Health Care Facilitator Program 

The Health Care Facilitator Program is a free and confidential service available to faculty, staff, retirees, survivors and eligible family members. They can help you solve complex problems with your medical, dental or vision plan. Health care facilitators are best used to handle complex problems that you can’t solve on your own. 

Areas of Assistance

 Health Care Facilitator Program staff are trained to handle issues such as:

  • Helping with billing problems for services which are covered by the health plan.
  • Providing a detailed comparison of your benefit options when you have a complex medical condition.
  • Intervening on your behalf with health plan representatives.
  • Explaining how Medicare benefits coordinate with UC-sponsored medical plans.
  • Completing the Request for Employment Information Form (CMS-L564 (PDF)) to enroll for Medicare Part B.

Tax Forms for 2025

To access your electronic tax documents, always go to your UCRAYS (for retirees) or UCPath (for current & former employees) account using a safe/known link. UC does not send any tax documents by email or text. If you receive an email or text with a link or an attachment for viewing your tax documents, it is a phishing scam designed to gain your private information. Do not open attachments or click email links that claim to provide access to your tax documents.   

Information for retirees

It’s easier than ever to access your tax documents from your UC Retirement At Your Service (UCRAYS) account. When you view your tax statement(s), you will be sent an email to confirm it was a valid request and you will be required to verify your identity by entering a one-time code sent to you via text or email.

If you need help logging in to your UCRAYS account, just call the UC Retirement Administration Service Center at 800-888-8267, Monday–Friday, 8:30 a.m. to 4:30 p.m. (PT), and select option 1. To minimize wait times, UnifyHR (a trusted UC partner) is providing additional support for UCRAYS issues.

1099-R statements

Select “Tax Statements” on your UCRAYS dashboard to view or download your 2025 tax statements. If you requested US Mail delivery, your printed form was mailed to the address you have on file as of January 2026; you also have the option to view a duplicate copy on UCRAYS.

Information for current and former employees

To access your tax forms on UCPath, click on Employee Actions > Income and Taxes > View Online W-2/W-2C and/or View Online 1095. If you have questions, please visit UCPath and click on “Ask UCPath Center” to submit an inquiry or call the UCPath Center at (855) 982‐7284 from 8 a.m. to 5 p.m. (PT), Monday through Friday to speak with an associate.

W-2 forms (Job Aid)

  • Current employees: UC employees now have access to their 2025 W-2 statements online.
    • Employees who opted for an electronic W-2 received an email notification.
    • Employees who did not opt-in for an electronic W-2 can still access their statement online via UCPath, and a printed W-2 statement will be sent to the mailing address listed in UCPath no later than February 2, 2026.
  • Former employees: can access and download their UCPath W-2 statements for up to three years after their separation date.
  • To access the W-2 statement, log in to UCPath and click on Former Employees. Once logged in, navigate to Income & Taxes and select the corresponding tile for the desired form.

Virtual Presentation: Health Insurance After You Retire

This presentation will explain UC’s retiree health and welfare benefits; which benefits can continue into retirement; eligibility for retiree health benefits; the cost of retiree health benefits and what UC requires of you when you become eligible for Medicare. The Health Care Facilitator will also provide resources for those who do not qualify for UC’s retiree health and welfare benefits.
