How to Complete a Self-Store Rental Agreement

Learn how to complete a UCSD Self-Store Rental Agreement form.

1. Get a Self-Store Rental Agreement form.

Download the Self-Store Rental Agreement (PDF).

2. Complete the top part of the Self-Store Rental Agreement.

List your department's:
  • Name
  • Index number being charged
  • Funding source and organization for the index number being charged
    • Note: Your department's index will be recharged monthly for storage charges.
  • Mail Code
  • List your name and phone number.
  • The contact person for Self-Store will complete the section marked For Office Use Only.

3. Read the middle part of the Self-Store Rental Agreement.

Read the information in items 1 – 5.

Note: UCSD Self-Store does not provide insurance for stored materials. To obtain insurance for your items, contact Kathleen Johnson, (858) 246-0369.

4. Complete the bottom part, left side of the Self-Store Rental Agreement.

  1. Have the Self-Store Rental Agreement form signed by the person in your department who has authority to sign for the index being charged.
  2. Print the authorizing person's last name.
  3. Fill in the date the form is signed.

5. The bottom part, right side of the Self-Store Rental Agreement will be completed by the Self-Store contact person.
