Registering Radiation Producing Machines

Principal investigators who acquire or fabricate a research (non-medical) radiation producing machine must register the machine with Environment, Health & Safety (EH&S). People who use the machine must be added to the registration for radiation producing machines.

Expand all

Register a machine.

  • Print and complete a Machine Use Registration form (PDF) (Word file).
  • Mail or fax the form to EH&S Radiation Safety:
    • Mail Code: 0089
    • Fax: (858) 822-7763

Add machine users to the registration.

  • Print and complete a Researchers Using Radiation Producing Machines (PDF) enrollment form for each individual listed as authorized personnel on the Machine Use Registration form.
  • Mail or fax the forms to EH&S Radiation Safety:
    • Mail Code: 0089
    • Fax: (858) 822-7763

Attend required safety training.

  • Make sure all people authorized to use the machine (including the PI) attend Radiation Producing Machines: Safety Fundamentals training via Enrollment Central.

Amend existing machine use registration.

  • Amend your machine use registration to report change of:
    • Project summary
    • Use or design of the machine
    • Machine location
    • Lab manager or contact
    • Survey meters
  • Print and complete a Machine Registration Amendment form (PDF) (Word file).
  • Mail or fax the form to EH&S Radiation Safety:
    • Mail Code: 0089
    • Fax: (858) 822-7763

Transfer, sell, or dispose of a machine.

  • Notify EH&S Radiation Safety, (858) 822-2494, prior to transferring, selling, or disposing of a radiation-producing machine.
