Controlled Substances: Managing Authorized Personnel

Principal investigators (PIs), learn how to update authorized personnel on your Controlled Substances Use Authorizations (CSUA), and review responsibilities and training.

Manage your CSUA authorized personnel

PIs must list all individuals accessing and working with controlled substances (CS) as authorized personnel on their CSUA.

  • To add authorized personnel:
    1. Complete a Form 3 – Personnel Screening Data Sheet for each individual.
    2. Send the completed and signed forms to the Controlled Substances Program Office:

  • To remove authorized personnel:
    1. Complete an Authorized CS Personnel List.
    2. Send the completed and signed forms to the Controlled Substances Program Office:
