Reporting a Lost/ Stolen Express Card or Fraudulent Charges

Use these step-by-step instructions to report an Express Card that has been lost or stolen, or to report unidentifiable or fraudulent charges to an account.

1. Contact the bank.

  • Gather this account information:
    • Express Card account number
    • Cardholder name
    • Employee ID number (in place of Social Security number)
  • For a lost or stolen card:
    • Contact U.S. Bank at (800) 344-5696.
    • Inform the bank representative that your "purchasing card" has been lost or stolen.
  • For unidentified or fraudulent charges:
    • Contact U.S. Bank at (800) 523-9078.
    • Inform the bank representative that unidentifiable or fraudulent charges were posted to your "purchasing card."

Note: U.S. Bank phone numbers are 24-hour, toll-free numbers.

2. Contact your Express Card Department Administrator (ECDA).

  • Inform your ECDA that:
    • Your Express Card was lost/ stolen or that unidentified/ fraudulent charges were posted to your account.
    • U.S. Bank was contacted to report the issue.
  • The Express Card Team will issue a new card for you. Additional administrator action is not necessary. The new card will be delivered via campus mail in 3–5 business days.

Note: UCSD is not responsible for charges that appear to your card after you've reported it lost or stolen. However, charges made prior to notifying the bank remain UCSD's obligation until a resolution is reached.

3. Complete the affidavit

  • U.S. Bank will mail you an affidavit, which you must complete and return within the allotted time frame in order to receive credit for the charges. A copy of this affidavit must be uploaded to each fraudulent transaction using the ECManager Attachment Tool.
