How to Report a Lost or Stolen Express Card

Use these step-by-step instructions to report a lost or stolen Express Card.

1. Contact the bank.

  • Gather this account information:
    • Express Card account number
    • Cardholder name
  • Contact U.S. Bank at (800) 344-5696.
    Note: This phone number is a 24-hour toll-free number.
  • Inform the bank representative that your "purchasing card" has been lost or stolen.

2. Contact your ECDA.

  • Contact your Express Card Department Administrator (ECDA) and inform him or her that your card has been lost or stolen.
  • Request that your ECDA cancel the card and request a new one.

3. Document unidentifiable charges.

  • If unidentifiable or fraudulent activity occurred on the account before the account was terminated, complete a Dispute form (Web) (Word).
  • Fax the completed form to:
    Disputes Representative
    Disbursements Division
    (858) 534-7194

Notes:

  • The Dispute form must be completed immediately upon determining fraudulent or unidentifiable charges. If the form is not processed within 60 days from the date of the questionable transaction, the University may not be able to receive credit for the transaction.
  • UCSD is not responsible for charges that appear on your card after you have reported it lost or stolen. However, charges made prior to notifying the bank remain UCSD's obligation until a resolution is reached with the vendor.
