How to File an Express Card Dispute

Follow these instructions to file an Express Card dispute.

1. When to file a dispute.

  • A dispute is filed for transactions that you have made. Common problems are: Billed for a different amount, duplicate billing, billed for merchandise you never received, or a credit not received.

2. Contact the merchant.

  • Contact the merchant to discuss the disputed item.
  • If you are unable to resolve the dispute with the merchant, proceed to Step 3.

3. Prepare the necessary form.

  • Complete and print the Express Card Dispute Form.
  • Be sure the cardholder's signature is included on the form. The form cannot be processed without the signature.

Note: This form must be submitted within 60 days from the date of the disputed transaction.

4. Submit the form.

  • Fax the form to (858) 534-4621.
    Attn: Disputes Representative

Note: A Disputes Representative will contact the bank. See the overview to learn about the actions the bank will take on your behalf.

5. File your copy.

  • File a copy of the completed dispute form for your records.
