If the payment is subject to reportable income, and the payee is a U.S. person, a W-9 form will be required when departments request that a new payee be set up for MyPayments.
The MyPayments payee set-up request form includes a link to download a W-9 form, and and provides a way to upload the form along with the request form submittal.
Departments can also obtain a W-9 (PDF), and submit the completed Fax Cover Sheet (Excel Version or PDF Version) and the W-9 to the fax number (this is a dedicated, secure fax line) on the cover sheet.
Note: Until further notice, this requirement does not apply to:
- New vendors being set up for purchase orders
- Foreign payees
The IRS requires a W-9 form to certify the taxpayer ID number, type of taxpayer (individual, partnership, or corporation), and the status as non-profit, exempt, etc. W-9s are required for payees who will receive payment subject to reportable income on the IRS 1099 form. Examples of reportable transactions include:
- Medical and health care payments (including corporations)
- Prizes and awards
- Research subjects
- Blood donors
- Professional services
- Guest lecturers
- Attorney fees
