The following tributes honor UC San Diego staff who passed away during their service with the university.
How to request a tribute
When a staff member passes away in your office, you may request to have a tribute posted using the Request for Staff In Memoriam on Blink (PDF).
- Consent from a family member is required. Electronic signatures or written consent are acceptable if the individual is unavailable to sign the form
- Completed requests should be emailed to hr-events@ucsd.edu
- Requests must be received by Human Resources within 12 months of the staff member's passing
