Complete the IRS-required form. |
- Download and complete the Independent Contractor Pre-hire Information Form (PDF).

Depending on the results of the form, take one of these steps: |
- If the form indicates a contractor relationship, go to Step 3.
- If the form indicates an employee relationship, see How to Hire Staff.
- If the form indicates an employee-vendor relationship, see Employee-Vendor Relationships.

Complete a High Value Purchase Requisition, regardless of the total cost of the service. |
- If you have a user ID and password, access FinancialLink to complete your High Value Purchase Requisition.
- See High Value Purchase Requisition for Personal Services for detailed information on completing your requisition.
Note: If you need a user ID, see How to Get a User ID. If another person in your area is responsible for preparing your requisition, provide that individual with the necessary information.

Submit documentation to Purchasing. |
- Include the following forms:
- Write the requisition number on all pages of documentation.
