How to Get a Biohazard Use Authorization (BUA) Verification Letter  
 
Summary: Grants applied for by principal investigators and their laboratory personnel often require verification of Biohazard Use Authorization (BUA). Follow these steps to get a BUA verification letter for funding agencies.

If you do not have a current BUA (approval granted within the last 11 months), read How to Open, Amend, and Deactivate a Biohazard Use Authorization (BUA).

What to do How to do it
1 Submit a request form.
  1. Print a BUA Verification Letter for Funding Agencies request form (Word) (PDF).
  2. Complete the form and have the applicant and the responsible principal investigator sign it.
  3. Send the form to the Environment, Health & Safety (EH&S) Biosafety Division:
    • Mail Code: 0089 (preferred method)
    • Fax: (858) 822-7763

    Important: EH&S needs at least 3 days to process the form.

  4. EH&S will prepare a verification letter signed by the UCSD Biosafety Officer. The letter will be sent to you according to instructions you provide on the request form.
