A Statement of Work contains essential information regarding the work that will be performed by a person or entity providing a service. It is required with High Value Purchase Requisitions for all types of services.
A Statement of Work must contain all of the following information:
- Project description: List the working name for the project and include all of the following where applicable:
- Prime sponsor
- Grant or contract number
- Principal investigator
- Project title
- Project scope: Describe, in detail, the services to be provided. Include:
- Any work plan
- Required deliverables
- Milestones
- Performance standards
- Due dates
- Period of performance: List the start and end date for the entire project.
- UCSD project manager or principal investigator and contact information: Provide the name of the faculty member, researcher, or administrator with oversight responsibility for the project, their campus address, phone and fax numbers, and e-mail.
- Vendor project manager and contact information: Provide the name of the vendor contact who has authority over the work, their address, phone and fax numbers, and e-mail.
- Physical location: Describe where the work will be performed.
- UC supplies and equipment: List the University-provided supplies and equipment that will be used.
- Payment rate: State the dollar amount cost of the project by deliverable, job, month, day, or hour. The preferred method is cost per deliverable.
- Total not to exceed: State the maximum dollar amount for the services.
- Payment terms: Provide any special terms of payment requested by the vendor.
- Employee-vendor relationship: Review Employee-Vendor Relationships, and disclose any relationship via the Employee-Vendor Disclosure Form (PDF), if necessary.
