Employees are eligible for service awards when they have achieved 10 or more years of service, at 5-year intervals (10, 15, 20, and more). The 2006-2007 ceremony will be held Thursday, Oct. 4, 2007, from 2:30 to 4:30 p.m. in the Price Center Ballroom.

Departments are responsible for submitting eligible employees' names and other required information. To do so:

• Verify that employees are eligible (see Eligibility, below).
• Download and complete the Service Awards form (Excel), including required information for all eligible employees.
  (Note: A Word file version is also available, but Excel is preferred.)
• E-mail the completed spreadsheet or table to worklife@ucsd.edu by Friday, Aug. 3, 2007.

Eligibility: Staff employees whose service achievement occurred between Sept. 1, 2006 and Aug. 31, 2007 (with Sept. 1, 2007 effective date) are eligible for this year's awards. To assist in determining eligibility, departments can use the Service Credit Calculation form (PDF), which should be submitted to Human Resources Records.
