Proof of Insurance  
 
Summary: Learn how to get proof of insurance when an outside supplier requests it.


When an outside supplier asks for proof that the University has general liability insurance, auto liability insurance, workers' compensation insurance, or property insurance, Risk Management provides a Certificate of Insurance.
  • Note: Professional liability insurance is verified by UCSD Medical Center Risk Management Office, (619) 543-6630 or (619) 543-6430.

To obtain a Certificate of Insurance from Risk Management:

  1. Contact the professional buyer for your area. The buyer will execute an associated contract or high-value purchase order with the outside supplier.

    Note: If the supplier's terms and conditions conflict with the University's business rules and policies, Purchasing will negotiate with the supplier. If a supplier won't change conflicting terms and conditions, Purchasing may suggest an alternate supplier. These actions may delay the purchasing process.


  2. When you receive the associated contract from Purchasing, send all the paperwork, including anything received from the outside supplier, to Risk Management at Mail Code 0925. Risk Management will verify all documentation, then send you the proof of insurance.

    Risk Management needs at least one week to process the documentation and send you the proof of insurance.

Notice: When a loss occurs, notify EH&S Risk Management, (858) 534-3820, immediately to promptly initiate the insurance investigation and claim process. Use pager number (858) 583-3302 for emergencies only. Take a picture if a camera is available.
