Get a Self-Store Rental Agreement form. |
- Download the Self-Store Rental Agreement (PDF).

Complete the top part of the Self-Store Rental Agreement. |
- List your department's:
- Name
- Index number being charged
- Funding source and organization for the index number being charged
- Mail Code
Note: Your department's index will be recharged monthly for storage charges.
- List your name and phone number.
- The contact person for Self-Store will complete the section marked For Office Use Only.

Read the middle part of the Self-Store Rental Agreement. |
- Read the information in items 1 – 5.
Note: UCSD Self-Store does not provide insurance for stored materials. To obtain insurance for your items, contact Scheryl Wade, UCSD Risk Management, (858) 534-4237.

Complete the bottom part, left side of the Self-Store Rental Agreement. |
- Have the Self-Store Rental Agreement form signed by the person in your department who has authority to sign for the index being charged.
- Print the authorizing person's last name.
- Fill in the date the form is signed.

The bottom part, right side of the Self-Store Rental Agreement will be completed by the Self-Store contact person. |

- Cage number
- Cage size
- Rate per month
- Access number
