How to do it |

Contact the bank. |
- Contact U.S. Bank by phoning the number that appears on the sticker located on the front of your card.
- Follow the prompts to activate your card.
When prompted for your Social Security number, provide your UCSD employee ID number.

Sign your card. |
- Sign the back of your card on the space provided.
