Contact the merchant.

- Contact the merchant to discuss the disputed item.
- If you are unable to resolve the dispute with the merchant, proceed to Step 2.

Prepare the necessary form. |
- Complete and print the Express Card Dispute Form.
- Be sure the cardholder's signature is included on the form. The form cannot be processed without the signature.
Note: This form must be submitted within 60 days from the date of the disputed transaction.

Submit the form. |
- Fax the form to (858) 534-4621.

File your copy. |
- File a copy of the completed dispute form for your records.
