How to File an Express Card Dispute  
 
Summary: Follow these instructions to file an Express Card dispute.
What you, the cardholder, should do How to do it
1 Contact the merchant.
  1. Contact the merchant to discuss the disputed item.
  2. If you are unable to resolve the dispute with the merchant, proceed to Step 2.
2 Prepare the necessary form.
  1. Complete and print the Express Card Dispute Form.

  2. Be sure the cardholder's signature is included on the form. The form cannot be processed without the signature.
Note: This form must be submitted within 60 days from the date of the disputed transaction.

3 Submit the form.
  1. Fax the form to (858) 534-4621.
    Attn: Disputes Representative

    Note: A Disputes Representative will contact the bank. See the overview to learn about the actions the bank will take on your behalf.

4 File your copy.
  1. File a copy of the completed dispute form for your records.
