How to Open, Amend, and Deactivate a Biohazard Use Authorization (BUA)  
 
Summary: Follow these steps to open, amend, or deactivate a Biohazard Use Authorization (BUA).

Research with biohazard materials must be approved by the Institutional Biosafety Committee (IBC). See Biohazard Use Authorization Overview for eligibility requirements.

Open a BUA
  • Use the online BUA Application. Read and follow the instructions on the Welcome screen to apply for a new BUA.
    • Choose the appropriate type of BUA: When you press the New BUA function button, 2 types of BUA applications are offered:
      • Research BUA for standard research
      • Clinical trial BUA involving human gene transfer (gene therapy)
  • Clinical trial (gene therapy) applications have special requirements. If you are applying for a clinical trial BUA, do the following:
    • Read the list of requirements (Word file) (PDF)
    • Contact EH&S Biosafety, (858) 534-6059 or 534-0353, for detailed formatting and submission instructions
    • Send the required material to EH&S Biosafety:
      E-mail: ehsbio@ucsd.edu (preferred method)
      Mail Code: 0089
      Fax: (858) 822-7763
  • Submission deadline: To ensure your BUA application is reviewed at the IBC's monthly meeting, submit your complete application no later than the 20th of the month.

Amend a BUA Amend your currently approved BUA prior to changing any research variable, or combination of variables, that may carry an increased biohazard risk. Read Biohazard Use Authorization (BUA) Overview for examples of when to amend a BUA.
  • Use the online BUA Application. Read and follow the instructions on the Welcome screen to amend an active BUA.
  • Clinical trial BUAs (gene therapy) have special requirements.
    • Contact EH&S Biosafety, (858) 534-6059 or 534-0353, for consultation and formatting instructions.
  • Submission deadline: To ensure your BUA application is reviewed at the IBC's monthly meeting, submit your complete application no later than the 20th of the month.

Deactivate a BUA Deactivate a BUA when the responsible PI retires, leaves UCSD, or no longer possesses biohazard materials.
  • Request BUA deactivation by notifying EH&S Biosafety in writing, referencing the BUA number:

  • Deactivations of clinical trial BUAs (gene therapy) have special requirements.
    • Contact EH&S Biosafety, (858) 534-6059 or 534-0353, for consultation.
